Choosing an HCBA

What is the Hidden Change Barriers Assessment?

HCBA helps you look more clearly at what may be getting in the way of a change, initiative, launch, or other organizational effort.

It is not a scorecard and it does not diagnose your organization. It uses the information you provide to prepare a bounded advisory readout: what the strongest supported pattern may be, what else could explain what you are seeing, and what may be useful to examine next.

Who is HCBA for?

HCBA can be useful for people responsible for change, transformation, implementation, operations, projects, technology, HR, leadership, or cross-functional work.

You do not need a change management background. The readout is written in plain professional language.

Do I need to know change management terminology?

No. Use the language you normally use to describe the situation.

You do not need to translate your experience into change management terms before answering.

Taking the Assessment

How long does the assessment take?

Most people complete the individual assessment in about 10 to 15 minutes.

If you choose to provide more detail in the written-response questions, allow about 15 to 20 minutes. You do not need to write long answers. Clear, specific answers are more useful than polished or lengthy ones.

Do I have to sign up for an account?

No. You do not need to create a password or set up a permanent account.

For an individual assessment, you provide the email address where you want your readout sent. For a team assessment, you use the shared team link and verify the work email address provided by the team coordinator with a short access code.

Should I include confidential or sensitive company information?

Only provide the context needed to describe the situation.

Do not include passwords, credentials, personal medical information, regulated personal data, trade secrets, or information you are not authorized to share. Names of individual employees are usually unnecessary. Roles, teams, conditions, and observed behavior are generally enough.

What happens after I submit the assessment?

Your readout starts preparing immediately. You will see an activity indicator, estimated progress, elapsed time, and a message showing that processing is continuing.

Most readouts are ready in about 30 to 60 seconds. Some may take a little longer. You do not need to submit the assessment again while it is processing.

What is included in my individual readout?

The exact content depends on what your responses support. A readout may include:

  • the strongest supported pattern,
  • what may be easy to miss,
  • what the pattern may mean,
  • signals to keep in view,
  • alternative explanations,
  • a more useful conversation to have,
  • the smallest useful next step,
  • leadership-ready framing,
  • language you can adapt,
  • what not to lead with, and
  • what the readout does not conclude.

A short assessment snapshot at the end reminds you of selected context the readout considered. It does not reproduce your full questionnaire.

Will the readout simply repeat what I entered?

No. Your responses are the evidence base, but the purpose of the readout is to add interpretation rather than reorganize your answers.

Where the readout refers to information you supplied, it separates what you told us from the interpretation that follows.

Why does the readout sometimes sound cautious?

Because HCBA is designed not to turn limited information into an unsupported root-cause claim.

A useful readout separates what the available information supports, what remains uncertain, what other explanations are still plausible, and what you could examine next.

Is the readout a diagnosis?

No. HCBA does not diagnose an organization, establish a root cause, decide that a person or group is responsible, or prove that one specific intervention is required.

It is a focused advisory readout intended to improve the next conversation and next decision.

Will my name or email address appear on my PDF readout?

No. Your name and email address do not appear on your PDF readout.

This is intentional. It allows the document to be saved or shared without automatically exposing your personal identifiers. A small support reference may appear in the footer of the final page.

The secure readout link is available for 7 days.

Download or save the PDF somewhere appropriate if you want to keep it longer.

Can I share my individual readout?

Yes, if you choose to.

Your name and email address do not appear on the PDF. The readout may still reflect information about your organization, so review it before sharing it more broadly.

Is my individual readout confidential?

Yes. Your individual responses and your individual readout are confidential.

Change Collaboration does not share your personal assessment responses or your individual readout with the team coordinator, your manager, other participants, or your organization without your authorization.

For a team assessment, participation status is tracked separately so the team coordinator can tell who has not started, who has started, and who has completed. That does not give them access to your answers or readout.

How is my information used?

Your information is used to provide the HCBA experience you requested.

For an individual assessment, your responses are used to prepare your readout and your email address is used to send it to you.

For a team assessment, your work email address is also used to confirm that you are an invited participant, send your access code, track participation status, and send your individual readout.

Your identifying information is not printed on your PDF readout. Your personal responses are not given to the team coordinator or other participants.

Does Change Collaboration share my assessment responses?

Not as part of the normal HCBA experience.

Your personal assessment responses and individual readout are not shared with other participants, the team coordinator, your manager, or your organization without your authorization.

For a team experience, protected team-level information may be used to provide the purchased team output. That does not mean giving anyone your individual questionnaire or individual PDF.

What can the team coordinator see?

The team coordinator can manage participation. They may see who was invited, who has not started, who has started, and who has completed.

They cannot see your individual answers, written responses, individual readout, or personal assessment interpretation.

Who is the team coordinator?

The team coordinator is the person managing the team assessment. They may be in HR, IT, change management, a project team, leadership, or another part of the organization.

They may purchase or set up the team assessment, provide participant email addresses, choose how invitations are sent, and see who has or has not completed the assessment.

They do not receive participants' individual answers or individual readouts.

Can the team coordinator also take the assessment?

Yes. If the team coordinator is also part of the team being assessed, they can participate like anyone else.

Their participant responses remain confidential. Being the coordinator does not give them additional access to anyone else's individual results.

What happens after I provide the participant email addresses?

You choose how the invitations will be sent.

If Change Collaboration sends them: each participant receives a polished invitation email. Everyone receives the same shared team link.

If you send them: you receive one shared team link and a ready-to-send invitation. You can forward the email, open a prewritten email, or copy the invitation into email, Teams, or another internal channel.

You never need to manage separate participant links.

Why do I provide participant email addresses if everyone gets the same link?

The shared link identifies the team, not the person.

When a participant opens it, they enter the work email address you provided for them. That lets the system confirm that they are part of the team, match them to the correct participation record, and track completion without showing you their answers.

Do I need to keep track of who has completed the assessment?

No. The system keeps track of participation.

The team coordinator can see whether each invited participant is pending, invited, started, or completed. You do not need a separate spreadsheet or a collection of individual links.

What happens when I open the shared team link?

You enter the work email address that your team coordinator provided.

If the address matches the participant list, you receive a 6-digit access code. Enter that code to open your confidential assessment.

Will the team coordinator know what I answered?

No. The team coordinator can see that you participated. They cannot see what you answered.

Will my manager see my individual readout?

Not through HCBA.

Your individual readout is sent to you. Change Collaboration does not provide your personal responses or individual readout to your manager, team coordinator, other participants, or anyone else without your authorization.

Can my responses be quoted in the team report?

The team experience is not designed to expose named participant responses.

The Team Pattern Readout focuses on protected team-level patterns, not a collection of attributed comments.

Does each participant get a different assessment link?

No. Each team engagement has one shared team assessment link.

Everyone in that engagement uses the same URL. Each participant verifies the work email address included in the team participant list.

What happens if Change Collaboration sends the invitations?

The team coordinator provides the participant email addresses during setup.

Change Collaboration sends a polished invitation directly to each participant. Every participant receives the same clean team link. Participation is still tracked separately for each person.

The team coordinator receives one polished email containing the shared team link.

They can forward the email as-is, open a prewritten message in their own email app, or copy a ready-to-send invitation into email, Teams, or another internal channel.

Why do participants need an access code?

Because everyone on the team uses the same shared link.

The access code confirms that the person entering the assessment controls the invited work email address and matches them to the correct participation record.

How long is the access code valid?

The 6-digit access code is time-limited. If it expires, return to the shared team link and request another code.

Can someone who was not invited use the shared link?

No. Having the link is not enough.

A participant must verify an email address that was included in that team's participant list.

Will participants receive long technical URLs?

No. Team invitations use one clean HCBA team URL rather than participant-specific token links.

What is a Team Readout Session?

Participants complete individual HCBAs and receive their own confidential individual readouts.

After the group has completed the assessment and the team experience is ready, Change Collaboration arranges a facilitated team conversation. This option does not include a written team-level analysis.

Does the team coordinator receive everyone's individual readouts?

No. Individual participant responses and individual readouts remain confidential.

The coordinator can see participation status, but not assessment content.

What is the purpose of the facilitated session?

The session examines protected team perspectives, clarifies the strongest useful pattern, and helps the group identify a practical next step.

It is not designed to expose or debate individual participant answers.

What is a Team Pattern Readout?

Participants complete individual HCBAs and receive their own confidential individual readouts.

After the group has completed the assessment and the team experience is ready, a written team-level pattern readout is prepared and delivered according to the distribution choice made during setup.

  • where perspectives align,
  • where the information remains mixed,
  • what the pattern may mean,

  • leadership-ready framing,
  • language the team can adapt, and
  • clear limits on what the report does not conclude.

Will the Team Pattern Readout identify who said what?

No. The team-level experience is not designed to attribute findings to named participants.

Individual responses and individual readouts are not included as named participant results.

What happens if the team information is mixed?

The readout may say that the available team-level information does not support one clear interpretation.

That is not a failure. It can be useful to know that the team does not yet share the same view of the situation.

How do you know when the team is finished?

The system tracks completion for the invited participant list.

Depending on the team setup, the team experience can move forward when everyone has completed the assessment or when the selected participation window closes and the team requirements are met.

What happens if someone never completes the assessment?

The team coordinator can see who is still outstanding and follow up if needed.

A team-level output will not expose the remaining participants' individual responses simply because someone did not participate.

Do you manage reminders when Change Collaboration sends the invitations?

Yes. When Change Collaboration is selected to send participant invitations, the team experience can support managed reminders for people who have not yet completed the assessment.

People who have already completed it are not treated as outstanding.

Can I complete the assessment on a phone?

Yes. HCBA is designed to work in modern web browsers on common desktop and mobile devices.

If you plan to provide longer written responses, you may find a larger screen more comfortable.

What if I close the browser while my PDF is being prepared?

It is best to keep the page open until processing finishes.

Your completed readout is also sent by email, so you do not need to keep the page open after successful completion.

Should I submit again if the readout is taking longer than expected?

No. The progress display shows that processing is continuing.

Repeated submissions are unnecessary.

What if I do not receive an email?

Check your spam or junk folder first. Then confirm that the email address used in the assessment or participant list is correct.
